Social determinants of health (SDOHs) are a wide array of social and economic factors, such as income inequality and unemployment, which have a tremendous impact on health outcomes. This webinar presentation will provide an overview of SDOH and the role of SDOH in health care. The presenters will also share and discuss the experience and preliminary findings from studying SDOH at Asian Health Services, a federally qualified community health center. Finally, presenters will demonstrate the use of CHC data within a software program that uses SDOH to adjust for patient risk.
